ALL NEW ENTRANTS to the school district will require:
A School Physical must be submitted within 30 days after entering school.
This exam must be completed, signed and stamped by a NY State physician
Immunization Record must be up to date, compliant, signed and stamped by an MD. If immunizations are in process, proof of an appointment to continue the immunization series must be given to the Nurse. New entrants require a vision, hearing, and color exam by our office.
